Routine screenings and examinations:

Students in the District are scheduled to receive mandated screening examinations in accordance with the following schedule:

Vision, growth and development (height, weight and BMI)

These are done every year- kindergarten through 12thgrade by the school nurse.  


Hearing:

This is done every year from kindergarten through 3rdgrade, then in 7thand 11thgrade.
Annually for all special education students and students with hearing problems.

Parents/Guardians receive notification from the nurse if the student does not pass any screenings done in school.


Physical Exams:

Public School Health Law requires all students in Kindergarten, 6thand 10thgrades to submit a completed physical exam by the first day of school.  Any exam done within a year will be accepted.


Dental Exams:

Public School Health Law requires all students in Kindergarten, 3rd and 7th grades to submit a completed dental exam by the first day of school.  Any exam done within a year will be accepted.

All new students entering Council Rock School District as transfers from other school districts are required to have current medical and dental examinations.


Medications:

All medications BOTH prescribed and over-the-counter must have a completed CRSD Medication Form by the doctor and parent or guardian if the medication is to be given in school.

*Medication must be sent to school in the original pharmacy container with the current prescription label. Upon request, pharmacists can prepare a duplicate container to be used for school.

*All medications must be brought directly to the health office. Students who have medications of any kind in their possession (in lunch boxes, school bags, etc.) may be considered in violation of the school district drug and alcohol policies and may be subject to disciplinary action. A parent or guardian, or other adult designated in writing by the parent or guardian, must transport all medications to and from school.

*A licensed registered nurse employed by the school district shall be the only district employee responsible for the administration of medications.

*If a licensed registered nurse is unavailable to administer the medication on a time schedule determined by the student’s physician, a care plan will be developed by the school nurse and the parent or guardian to ensure that the dosage is administered as scheduled.

*All medications are kept in the health office in a locked cabinet.
*Acetaminophen, for which the district has a standing order from the district physician, will be administered with the signed permission of parent or guardian as noted on the emergency card.


Immunizations:

Pennsylvania legislation requires that all children at any grade, kindergarten through 12th, including all public, private, parochial, intermediate unit and home schooled students, show proof of immunization before they can attend school in the Commonwealth, unless an exemption for medical or religious reasons is granted:

The following minimum immunizations are required at all grade levels:

Diphtheria and Tetanus - Three or more properly spaced doses of DTP, DtaP, Td or DT, or any combination of the three.

A fourth diphtheria-tetanus immunization (given as DPT, DtaP,Td, or DT) administered after the child’s fourth birthday.

Polio - Three or more properly spaced doses of polio vaccine, (IPV or OPV).
Measles (Rubeola) - One dose of live attenuated measles containing vaccine (preferably MMR) administered at 12 months of age or older, AND a second measles immunization (preferably MMRII) (minimum of 24 days after the first dose), or measles immunity proved by serological testing.

German Measles (Rubella) - One dose of live attenuated rubella containing vaccine (preferably MMR) administered at 12 months of age or older or rubella immunity proved by serological testing.

Mumps - One does of live attenuated mumps containing vaccine (preferably MMR) administered at 12 months of age or older.

Hepatitis B -three properly spaced doses of hepatitis B vaccine.

Varicella - proof of varicella (chickenpox) immunization or a written statement of having had the disease. By September of  2011/2012 school year, 2 doses of Varicella vaccine will be required.


Communicable Diseases Requiring Exclusion

If a communicable disease is suspected, the parent will be notified and the child will be excluded from school. The student will not be readmitted before the minimum number of days designated by the State and Local Departments of Health which are:

Diphtheria - 2 weeks from the onset or until appropriate negative culture tests.

Chicken Pox - 5 days from onset of first crop of vesicles (blisters) or when all blisters are dried and crusted.

Rubella (German Measles) - 4 days from onset of rash.

Rubeola (Regular Measles) - 4 days from onset of rash.

Mumps - 9 days from onset or until subsidence of swelling.

Pertussis (whooping cough) - 3 weeks from the onset or five days from institution of appropriate therapy.

Respiratory streptococcal infections (including Scarlet Fever) - Not less than 10 days from the onset if no medication is prescribed or 24 hours from the institution of appropriate therapy.

Infectious Conjunctivitis (Pink Eye) - Until judged non-infective; that is, without a discharge.

Ringworm - Immediately after therapy is instituted, if body lesions are covered. Neither scalp nor body lesions that are dried need to be covered.

Impetigo Contagiosa - 24 hours from institution of appropriate therapy.  

Pediculosis Corpora (Body Lice)- After completion of appropriate treatment and judged non-infective by the school nurse or child's physician.

Scabies - After completion of appropriate therapy and until judged non-infective by the school nurse or child’s physician.

Trachoma - 24 hours from institution of appropriate therapy.

Tuberculosis - Following a minimum of two weeks of adequate chemotherapy and three consecutive sputum smears. In additional, a note from the physician that the child in noncommunicable shall be submitted prior to readmission.

Neisseria meningitidis - Until judged non-infective after a course of rifampin or other drug which is effective against this disease, or otherwise shown to be non-infective.


Emergency Information

Since the emergency information  is essential for maintaining contact with parents and guardians, it is recommended that parents and guardians notify the school promptly when there are changes in home or business telephone numbers.
